If you have a pet who needs to be seen by our doctors, please make a Patient Portal account, and call us as soon you arrive at our hospital. You will only need to do this once! After that, you only need to let our reception team know you have an account in our system and they can put you on our schedule!

The portal will ask for your Full Name, Phone Number, Mailing and Email Address, then it will let you add as many pets under your name as you want! It will ask the Name, Breed, and Age of each pet, and then both of you will be ready to go when you arrive.

Once you and your pet arrive at our hospital, our reception staff will be able to add you to our schedule.
You can also "book an appointment" on your own through your account, but please keep in mind our hospital is an emergency room and patients who are the most critical must be tended to first regardless of when they arrive. We are unable to guarantee exact time spots through our website, but you are welcome to request an urgent care exam and we will fit you in as soon as we possibly can.

If you would like to complete all the paperwork ahead of time, below you will find the medical history questions we ask about your pet when they need to be seen.

We are often able to get you checked in sooner if we have all of this information for your pet and their current and past medical issues, but it is more important for you to have a patient portal with us. We can ask these questions and fill this out with you when you arrive if you would prefer.
